Children's Case Manager/Care Coordinator (Portland) - $500 Sign On Bonus

We are looking for a Care Coordinator to join our Children's Behavioral Health Home team!
In this model, you will be an integral part of a team-based approach to children's case management which also includes a Clinical Team Lead, Family/Youth Support Partner, Nurse Care Manager, and Psychiatric and Medical Consultants.
This is a highly collaborative role that addresses the behavioral health and medical needs of youth.

$500 Sign on Bonus!!
Many positions include sign on bonuses that are available to full-time newly hired staff. This sign on bonus ($500) will be paid after 90 days of full-time employment. If a new hire changes status to either part-time or per diem status during any of that timeframe, they will forfeit the sign on bonus that is yet to be paid.
Duties of the position include conducting psychosocial assessment, treatment planning, connecting youth and their families with needed resources, addressing barriers, and advocacy. Interdisciplinary collaboration and strong networking skills are essential, as is flexibility to travel to homes to meet with families as needed. Further responsibilities include submission of Kepro managed care entries, clinical documentation and electronic record keeping.
This is a multi-faceted position, where you will need to be highly organized and able to juggle tasks, prioritize needs, and complete required documentation within each week. Care coordinators need to be tech savvy, as you will work within various computer programs.
Bachelor’s degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social service and working with children and families is preferred.
