Spurwink's Lewiston office is looking for a Care coordinator/Case manager to join our Children's Behavioral Health Home team!

In this model, you will be an integral part of a team-based approach to care which also includes Clinical Team Leader, Family/Youth Support Partner, Nurse Care Manager, and Psychiatric and Medical Consultants. 
 
This is a highly collaborative role that addresses the mental health, behavioral and medical needs of the client.

Our program works closely with many Lewiston and neighboring schools, as well as other community providers. You will also work closely with other Spurwink programs and services.
 
This is a great opportunity to work with a diverse population of families within this immediate Lewiston community, and beyond. Our office covers 4 counties: Androscoggin, Sagadahoc, Franklin and Oxford.

 Salary Range: $16.50-$18.50 - Based on experience with additional individual and team based incentives.

 

$1000 Sign on Bonus!!

 

Many positions include sign on bonuses that are available to full-time newly hired staff.  The first half of the sign on bonus ($500) will be paid after 90 days of full-time employment and the second half of the sign on bonus ($500) will be paid after six months of full time employment.  If a new hire changes status to either part-time or per diem status during any of that timeframe, they will forfeit the sign on bonus that is yet to be paid.

 

 

Duties:

You will connect families with needed resources, and help address and work through any barriers. This is a community based position, you will be working with families in their homes, although families are also welcome to come meet in our office. 

As a Care Coordinator, you will assess needs with the family during intake, develop treatment plans, monitor services and are responsible for collaboration between providers.

This is a multi-faceted position, where you will need to be able to juggle tasks, prioritize needs of the client as well as meet family needs, and complete required documentation within each week. Care coordinators need to be tech savvy, as you will work within various computer programs.

 

 

Qualifications:

Bachelor’s degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social service preferred.
