Adult Community Case Manager $1500 Sign on Bonus!!

Spurwink is seeking an organized, energetic and dynamic individual to join our growing Adult Community Case Management Team. This is a full-time position, covering primarily Kennebec and Androscoggin counties, with a primary work location out of our Randolph office. Regular travel is required for this position .
$1500 Sign on Bonus!!
Many positions include sign on bonuses that are available to full-time newly hired staff. The first half of the sign on bonus ($750) will be paid after 90 days of full-time employment and the second half of the sign on bonus ($750) will be paid after six months of full time employment. If a new hire changes status to either part-time or per diem status during any of that timeframe, they will forfeit the sign on bonus that is yet to be paid.
Typical and required duties include: Provide initial assessment when clients are referred; Complete intakes, administer assessment screenings as required, and enter data into electronic data base for client records; Maintain regular contact with client, guardian, and other team members and community professionals involved with client; Facilitate Person Centered Planning (PCP) team meetings and develop a plan of care (PCP) for the client with input from the team. Work closely with team members to oversee implementation of the Person Centered Plan. Monitor and document progress; Ensure that clients (and guardians/families, as applicable) are aware of relevant community services and resources, and assist clients in accessing resources; Work effectively with other programs inside and outside of Spurwink to ensure effective communication and best possible outcomes for clients; Maintain records and documentation as required by Spurwink and OADS.
Minimum Requirements: Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field, or degree in a related field with a minimum of one year of experience working with individuals with intellectual/developmental disabilities.
