Case Manager/Care Coordinator for BHH - $1000 sign on bonus!

We are looking for a care coordinator/case manager to join our Children's Behavioral Health Home team!
This position will be unique in that you will split your time between the Lewiston and the Randolph offices. You will work geographically between those two regions as well as being located in both offices during the week. In this model, you will participate in a team-based approach to care, by working in collaboration with Clinical Team Leader, Family/Youth Support Partner, Nurse Care Manager, and Psychiatric and Medical Consultants.
As a Care Coordinator, you will work with children and their families to address behavioral health needs, as well as integrating physical health/medical needs. You will work with a diverse population of families.
$1000 Sign on Bonus!!
Many positions include sign on bonuses that are available to full-time newly hired staff. The first half of the sign on bonus ($500) will be paid after 90 days of full-time employment and the second half of the sign on bonus ($500) will be paid after six months of full time employment. If a new hire changes status to either part-time or per diem status during any of that timeframe, they will forfeit the sign on bonus that is yet to be paid.
Care coordinators assess needs with the family during intake, develop treatment plans, monitor services and are responsible for collaboration between providers. You will connect families with needed resources, and help address and work through any barriers. This work is mostly home and community based.
Our program works closely with local schools, as well as other Spurwink programs such as day and residential services, Child Abuse Program and there is some potential to work closely with Spurwink's Failure to Thrive program.
Care coordinators need to have strong time management skills to manage family and documentation needs. Care coordinators need to be tech savvy as you will work within various computer programs. You will need to be able to juggle tasks, prioritize needs, meet family needs, as well as complete required documentation within each week.
Bachelor’s degree from an accredited four (4) year institution of higher learning with a specialization in social services or closely related field. Experience in social service preferred.
