Lead Adult Community Case Manager $1000 Sign on Bonus

Spurwink is seeking an organized, energetic and dynamic individual to join our growing Adult Community Case Management Team, with additional duties related to promoting and marketing the program. The position is full-time, covering York and Cumberland Counties. Regular travel is required for this position.
$1000 sign on bonus.
Many positions include sign on bonuses that are available to full-time newly hired staff. The first half of the sign on bonus ($500) will be paid after 90 days of full-time employment and the second half of the sign on bonus ($500) will be paid after six months of full time employment. If a new hire changes status to either part-time or per diem status during any of that timeframe, they will forfeit the sign on bonus that is yet to be paid.
Typical and required duties include: Provide initial assessment when clients are referred; Complete intakes, administer assessment screenings as required, and enter data into electronic data base for client records; Maintain regular contact with client, guardian, and other team members and community professionals involved with client; Facilitate Person Centered Planning (PCP) team meetings and develop a plan of care (PCP) for the client with input from the team. Work closely with team members to oversee implementation of the Person Centered Plan. Monitor and document progress; Ensure that clients (and guardians/families, as applicable) are aware of relevant community services and resources, and assist clients in accessing resources; Work effectively with other programs inside and outside of Spurwink to ensure effective communication and best possible outcomes for clients; Maintain records and documentation as required by Spurwink and OADS; Promote and market Spurwink’s Adult TCM program by networking and collaborating with agency stakeholders and community partners. Attend community meetings with or as assigned by Program Manager or Regional Director; Provide training and coaching to peers as delegated by Program Manager or Regional Director.
Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field, or degree in a related field with a minimum of 2 years full-time relevant human services experience.
Minimum of two years of experience working in the developmental disabilities field required.
